Chronic Disease Specialist

Assist in promoting and restoring patient’s health telephonically by contacting patients for direct services with healthcare providers and multidisciplinary team members and provide chronic disease tracking/ advisement. Schedule & manage appointments, advise patient on specimen collection techniques, follow-up on test results, assist patient with all questions for their healthcare.

Qualifications:

To perform this job successfully, an individual must be able to perform each medical office duties which he/she is capable of by virtue of appropriate training and with the scope and limits of the Medical Assistant/Certified Nursing Assistant, perform each essential duty listed above satisfactorily and with professionalism and courtesy to others, able to effectively and efficiently organize and carry out workload. Must be bilingual in English/Spanish.

At least six months in an office environment, High School diploma or GED, Diploma from an accredited Medical Assistant school or a Certified Nursing program. Must have a valid CPR card. Either currently have a AZ Fingerprint card or submit for one within 5 days of hire
